How the Process Works

1. Initial Contact & Scheduling

The process begins when you contact our office by phone or online booking.

During scheduling, you will:

  • Provide basic contact and insurance information
  • Receive intake forms to complete before your appointment

New patient psychiatric evaluations typically last longer than follow-up visits to allow for a thorough assessment.

2. Before Your Appointment

You will complete intake paperwork covering:

  • Substance use (if applicable)

If the evaluation is for a child or adolescent, a parent or legal guardian will complete the forms and participate in the visit.

3. During the Psychiatric Evaluation

The evaluation involves a structured clinical conversation. This may include:
